RESCUE IC Flip Chart for use with Amendment 1 [PDF]
For use during the informed consent process as a visual aid for the patient being consented to the study.
Please note: The desktop flip chart display binders are provided by ACRIN.

Letter to Physicians [DOC] - This letter can be sent from the site's principal investigator to any physicians
they hope to have refer patients to the study. Please note: The "Letter to Physician" is intended as a draft
for sites to copy onto their letterhead and is not included on the order form.

Eligibility Card for use with Amendment 1 [PDF]
To be distributed to all clinicians who may have an opportunity to identify study participants and/or placed
in a convenient location for easy clinician access. Eligibility criteria are listed on one side and a body mass
index table is on the reverse.

Suggested OMT [PDF] - To be distributed to all clinicians who will have an opportunity to discuss
"optimal medical therapy" with study participants as a helpful reminder of OMT specifics.

Patient Card Example [PDF] - To be given to each participant as a study reminder, and to help them
inform non-study doctors about their RESCUE participation.

RESCUE Poster [PDF] - For display in areas likely to be seen by cardiac patients, as well as in general
high traffic areas inside or outside your facility.
